September’s household budgets are already noisy: new school costs, the last warm-weather boarding weekend, the first indoor-heating vet visit for a cough. That is a real seasonal reason to understand *how* a claim pays, not a fabricated “insurance crisis.” A deductible and a reimbursement percentage decide whether a $2,000 invoice becomes something you can float on a card. We will walk through the mechanics using only the six U.S. pet insurers Pet Standings has verified from public pages: Embrace, Healthy Paws, Trupanion, Fetch, Lemonade, and Nationwide.

We are the Pet Standings editorial team. We are not claims adjusters. The sample policy for your state wins every argument with this essay.

Deductible: the amount you absorb first (but “first” is not the same everywhere)

A deductible is the portion of eligible costs you pay before the insurer’s reimbursement formula applies. It is not the same as a copay at a human clinic, and it is not the exam fee.

Annual deductible. You meet it once per policy year across eligible claims, then reimbursement applies to later eligible costs until the year resets. If the year resets before a chronic condition is “done,” you can meet a deductible again. Several customizable accident-and-illness brands (Embrace, Healthy Paws, Fetch, Lemonade, Nationwide modules on public pages) let you pick a deductible amount. Higher deductible, lower premium — that relationship is standard. We will not invent a chart of “average U.S. deductibles.”

Per-incident deductible. Some forms apply a deductible each time a new incident occurs. Two unrelated injuries in one year can mean two deductibles. This is why the word “incident” in a sample policy is worth a highlighter.

Lifetime per-condition deductible (Trupanion). Trupanion’s public FAQ describes paying the deductible once per condition. After that, eligible costs for that condition continue without meeting a new annual deductible for the same condition. A second, unrelated condition can have its own deductible. That design is different from “$500 per year for everything.” It can be better or worse depending on whether you have one expensive chronic diagnosis or many small unrelated issues. We are describing the public design, not predicting your household.

Eligible versus ineligible matters. If a line is excluded (pre-existing, waiting period, food, a wellness item on a medical policy), it usually does not chew down the deductible. People are surprised when a big invoice barely moves the deductible because half the lines were never eligible.

Reimbursement: the percentage after the deductible

If a policy reimburses 80% of eligible costs after the deductible, you owe 20% of those eligible costs plus everything excluded. Embrace’s cost page describes 70%, 80%, and 90% options. Healthy Paws describes reimbursement up to 90%. Trupanion describes up to 90% of eligible costs. Fetch marketing describes up to 90%, with a common published configuration of 80%. Lemonade lets you customize; independent coverage has noted that Lemonade may apply coinsurance before the deductible, which changes the arithmetic versus a deductible-then-percentage formula. Read Lemonade’s current policy. Do not assume every brand subtracts in the same order.

A worked example with round numbers — illustrative only, not a quote from any of the six companies:

Suppose eligible veterinary costs are $1,000, the deductible remaining is $200, and reimbursement is 80% after deductible. You pay $200 deductible plus 20% of the remaining $800, which is $160, totaling $360, and the insurer would reimburse $640 of eligible costs. If $300 of the invoice was ineligible, the eligible amount was never $1,000. That is the usual disappointment, not a secret “denial rate” we invented.

Annual limits sit on top of both

Healthy Paws: $5,000, $7,000, or unlimited, chosen at enrollment. Trupanion: no annual or lifetime payout cap on eligible costs, per the official site. Fetch: unlimited not verified; $10,000 is a common configuration they disclose. Lemonade: customizable limits, unlimited not offered according to the 2026 NerdWallet review we cite. Embrace: customizable, with an unlimited option described on public comparison pages. Nationwide: modular examples include specified limits (the $12/month accident-only footnote uses a $5,000 limit in that sample).

Once you hit an annual cap, reimbursement stops even if you have met the deductible. Unlimited does not erase exclusions.

Exam fees, extras, and why the invoice has 20 lines

Fetch says sick-visit exam fees to diagnose a covered injury or illness are in the base accident-and-illness plan. Embrace describes exam fees as an optional add-on. Lemonade lists vet visit fees as an add-on. If exam fees are not covered, they may never enter the deductible math.

Wellness riders (Embrace Wellness Rewards, Fetch Wellness, Lemonade preventive add-ons, Nationwide wellness) usually have their own schedules and are not the same pool as accident-and-illness reimbursement. Do not expect a teeth-cleaning stipend to pay for a foreign-body surgery.

Claims path: reimbursement versus the front desk

Reimbursement. You pay the hospital, submit invoices, wait. Healthy Paws states that most claims are processed in two days — the company’s claim. Embrace markets fast claims; we did not time them. Fetch uses digital claims without an Express-style SLA we verified.

Direct pay. Trupanion Express can pay participating hospitals at checkout so you owe your deductible and coinsurance there. This is an ease feature, not a different deductible formula.

App instant pay. Lemonade says 50% of eligible claims pay instantly in the app, with the rest averaging within five days. Eligibility still depends on the policy.

Nationwide’s public site describes visiting any licensed U.S. veterinarian; it does not, on the pages we used, document an Express-style or instant-app SLA.

Criteria to set before you compare monthly prices

  1. Choose a deductible you could pay twice in a bad year, not the highest number that makes the premium look like a streaming subscription.
  2. Pick reimbursement (70/80/90 where offered) knowing you still pay the remainder.
  3. Decide whether an annual cap is acceptable. If you want unlimited as a verified option, Healthy Paws and Trupanion are the clear public designs; Embrace describes an unlimited option; Fetch/Lemonade/Nationwide should be read as capped or unverified-unlimited.
  4. Decide whether exam fees must be in the base plan (Fetch’s description) or can be an add-on.
  5. Ask how coinsurance and deductible are ordered — especially on Lemonade.
  6. Confirm participating clinics if you want Trupanion Express.

Who this math is for

If you have an emergency fund that already covers a five-figure vet invoice, insurance is optional risk transfer. If you do not, the deductible is the amount you must still be able to pay on a Tuesday night. Comprehensive insurance with a deductible you cannot meet is a paper policy.

Senior pets and pre-existing conditions complicate eligibility more than they change the arithmetic. The formula only runs on eligible lines.

Disclosure

Pet Standings may earn a commission if you buy through links on our insurance ranking. We still refuse to invent a “national average reimbursement.” Premiums vary. Get two quotes from the six verified brands using the same deductible and reimbursement, then read both sample policies.

Submitting a claim without folklore

Photograph the itemized invoice, not a credit-card receipt. Keep the medical notes. If the hospital uses different names for the same procedure, the policy’s eligibility rules still apply to the diagnosis, not the marketing name of the package. Direct-pay (Trupanion Express) still requires the clinic to participate. Instant app payment (Lemonade) still requires an eligible claim.

If a claim is delayed, ask whether the delay is missing records, a waiting period, or a coverage decision. Those are different problems. We did not independently audit each company’s average turnaround beyond quoting their own public statements (Healthy Paws’ two-day processing claim; Lemonade’s instant 50% claim).

Changing deductibles later

Some companies lock annual limits or unlimited choices at enrollment (Healthy Paws: unlimited cannot later be increased if you picked a cap). Deductible changes at renewal are a policy-specific rule. Assume nothing. Ask in writing. A lower premium next year may mean a higher deductible you no longer remember choosing.

Disclosure again, because the math is where people get hurt

We may earn a commission if you click through our ranking. That is not a reason to pick the highest reimbursement without being able to pay the deductible on the same day the surgeon wants a deposit. Compare the six verified plans, then run one example invoice through each sample policy’s order of operations.
